What B12 does
B12 injections are one of the most-sold items in the wellness world, and one of the most oversold. So here is the honest version up front.
If you are low in B12, correcting it can make a real difference. If you are not, an injection will do very little.
That is not a reason to avoid it. It is a reason to find out which you are first — and it turns out a meaningful share of adults over 50 fall into the first group without knowing it.
Vitamin B12 is essential to two things you would notice if they went wrong: making red blood cells, and maintaining the myelin sheath around your nerves. It is also a cofactor in energy metabolism.
When it is low, the effects build slowly and do not announce themselves as a vitamin problem. Fatigue is the common one. So are brain fog and difficulty concentrating, low mood, and — as deficiency progresses — tingling or numbness in hands and feet, and problems with balance.
The nerve symptoms matter most, because prolonged deficiency can cause damage that does not fully reverse. That is the real argument for checking rather than guessing.

Who is actually likely to be low
This is the useful part, because several of these describe our patients directly.
Adults over 50. Absorbing B12 from food requires stomach acid and a protein called intrinsic factor, and both decline with age. This is the single most common reason midlife adults run low, and it happens regardless of diet.
Anyone on long-term metformin. Metformin is associated with reduced B12 absorption, and B12 monitoring is a recognized part of ongoing metformin care.
Anyone on acid-reducing medication. Proton pump inhibitors and H2 blockers reduce the stomach acid absorption depends on. Many people have taken these for years without connecting them to fatigue.
Vegetarians and vegans. B12 occurs almost exclusively in animal foods.
Anyone with a malabsorption condition or previous gastric surgery — celiac disease, Crohn's, bariatric surgery.
If none of these apply and your levels are fine, an injection is unlikely to change how you feel. We would rather say that than sell you a course of them.

The honest summary: the question is not injection versus oral, it is why you are low. If the problem is absorption — which is the common one after 50 — an oral supplement has to cross the exact barrier that is not working, and injection is the route that gets around it. If the problem is dietary intake in someone who absorbs normally, an oral supplement is usually sufficient and much cheaper.

Injection-site reactions are the most common effect and are usually minor. B12 is water-soluble and generally well tolerated.
Tell your clinician if you have a history of kidney disease or Leber's hereditary optic neuropathy, and mention any other B-vitamin supplementation you are taking.

An honest word on the evidence
B12 supports normal energy metabolism; benefits are clearest when levels are low. To be direct about the two ends of that: where levels are genuinely low, correcting them can produce a clear improvement in energy and cognition and can halt the progression of nerve symptoms — this is well established. Where levels are already adequate, additional B12 is excreted rather than stored to any useful effect, and the energizing sensation some people report after a shot is not well supported by evidence. One nuance worth knowing: standard B12 reference ranges are wide, and some people have symptoms at the lower end of “normal.” That is a real clinical judgment rather than a marketing claim, decided with a clinician looking at the whole picture rather than one number.
